When should I arrive and what should I bring with me to my first physical therapy visit?

Please arrive at the clinic 20 minutes early to fill out your detailed patient information forms. If you would like to fill out the form before arriving, it is available below to download.

What Should I Wear?

Since physical therapy involves movement and exercise, it is recommended that you wear clothes that are loose fitting and allow for easy movement. If your lower body is involved, please wear shorts. If we will be treating your shoulder or neck region, ladies are recommended to wear a tank top or sports bra. Athletic or walking shoes are preferred!

What should I expect at my first physical therapy visit?

Be prepared for a detailed overview and discussion of your injury and symptoms. After a thorough examination, we will set up the best and fastest plan for a full recovery. We provide pain relief and hands on treatment which includes a simple and concise home exercise program.

Direct Access / Self-Referral

Direct Access references the ability for someone to see a physical therapist without referral from a physician. Many insurance companies now recognize the cost benefit of allowing patients to self-refer to physical therapy clinics. If you are experiencing pain and discomfort from a commonly treated injury, you may not need a referral from your doctor. Please contact us for more information!
